Time’s running out! Health assessment deadline June 15

Time’s running out to take a free, confidential health assessment by June 15 to learn how your lifestyle habits affect your health. Then do a follow-up activity or take the follow-up wellness coaching to earn a $100 gift card. If you’re a supervisor, remind your team about this opportunity.

Once you complete the health assessment, you receive immediate personalized feedback to improve or maintain your health. Additional information and resources are also available to help you make lifestyle choices that will enhance your quality of life.

Through StayWell, UC offers 50 points to employees who complete the health assessment by Saturday, June 15, 2013. If you complete follow-up activities (worth another 50 points) by Dec. 15, 2013, you are eligible for a $100 incentive award gift card. Spouses and domestic partners eligible for the StayWell program can receive a $50 gift card.

It’s easier than ever to earn points toward your incentive award. You can earn your follow-up points by attending certain campus-sponsored wellness events, such as last month’s UC Walks. (If you participated in UC Walks, see these instructions to obtain your 25 incentive award points through the StayWell program.) Or, take advantage of StayWell wellness coaching and online programs.

The program is completely voluntary. You must have been enrolled in an eligible UC medical plan on Jan. 1, 2013, to qualify for the 2013 award.

Kaiser Permanente members and some employees represented by a union are not eligible for the StayWell program. Kaiser members can participate in Healthworks, Kaiser’s health assessment and follow-up program.
