Time is running out: Health assessment due June 15
June 15 is the deadline to complete your StayWell health assessment if you want to be eligible for the StayWell incentive award.
Eligible employees who complete a health assessment and follow-up activity are eligible to receive a $100 gift card, and spouses or domestic partners a $50 gift card. Even if you completed a health assessment and received a gift card in 2011, you may participate again this year.
The cards may be used at dozens of retailers, restaurants, specialty shops and online stores as well as for donations to several charities.
The StayWell health assessment is a tool for learning how your lifestyle habits can affect your health and well being. After you complete a confidential online questionnaire – a process that takes about 10 minutes – the StayWell program immediately provides you with a personalized health report.
The report can tell you about health risks you didn’t know you had; offer information on relevant topics such as weight management and high blood pressure; and give you tools and guidance for making positive health behavior changes, including follow-up wellness coaching, if appropriate.
The university offers the health assessment as part of its commitment to the well being of its employees and family members. Your personal health information and the StayWell assessment remain private. UC never receives any individual health information about StayWell participants. You also control whether to share your health assessment with your medical plan.
Employees must have been enrolled in a UC medical plan (except Kaiser) on Jan. 1, 2012, to participate. Kaiser members are eligible to participate in the Kaiser health assessment program. Represented employees should check to see if their union has agreed to participate in the StayWell program.
