Diabetes prevention and management resources from UC’s health plans
About one in 10 American adults has diabetes, and one in three has prediabetes, according to the Centers for Disease Control and Prevention (CDC). Awareness, prevention and management are key to avoiding the serious complications associated with type 2 diabetes, including heart attack, stroke and blindness.
If you or a family member could use support, your UC health plan is a good place to start. All of UC’s medical plans include preventive care at no cost to you, including screenings for diabetes and other health risks. UC’s health plan partners also offer tools and programs specifically designed to help their members prevent and/or manage diabetes.
Anthem Blue Cross
Take a one-minute quiz to find out if you qualify for Solera, Anthem’s free diabetes prevention program. Most programs include small group support, access to a personal health coach and tools like a wireless scale or an activity tracker.
Kaiser Permanente
Kaiser members with diabetes can find support through the customized Care for Diabetes program. Complete a questionnaire and you’ll receive an action plan and tools matching your preferences, your individual ability and your level of commitment.
Health Net
Health Net offers UC members access to Omada, an online program that helps individuals reduce their risk for serious, preventable diseases, including type 2 diabetes and heart disease. Omada promotes healthy choices and lifelong habits through resources such as access to health coaching, a wireless scale and digital pedometer, weekly interactive lessons and an online support group.
Western Health Advantage
Diabetes virtual coaching is a self-paced coaching program for Western Health Advantage members managing diabetes or prediabetes. The program includes a personalized action plan with tools to help you track your progress, along with healthy recipes and educational articles, videos and podcasts.
